Dr Peter McCullough, leading Covid vax critic, has turned his attention for an instant, to the issue of the possible relationship between autism and gender dysphoria, people feeling that they have been born in the wrong sex. Both autism and gender dysphoria have been on the increase in recent years, but is there a link? There has not been much research on this question, but one paper found that

20 percent of people with gender dysphoria also suffered from autism. The question to be investigated is if there is a direct causal link, or some common pathophysiological determinants.

The CDC indicates that 1 in 36 children has been identified with autism spectrum disorder (ASD) according to estimates from CDC’s Autism and Developmental Disabilities Monitoring (ADDM) Network. This represents a sharp increase up from 1 in 10,000 in 1970. There has also been a less well delineated rise in gender dysphoria (GD) which is driving the clinical development of transgender medicine.

A natural question is: could ASD and GD be linked? While the epidemiology of both conditions is not well developed, several papers have reviewed the available evidence and have found a link. Van Der Miesen et al, in a narrative review concluded that about 20% of GD patients indeed have ASD.

When large, well performed epidemiological studies are completed it would not be a surprise if ASD is found to be an independent risk factor for GD. More complex analyses will be needed to understand if both conditions, which co-exist, have common pathophysiological determinants.

So the next time there is a news story on transgender rights, surgical advancement, hormone therapy, and psychological outcomes, keep in mind that autism is likely playing a role in the interpretation of what is portrayed to the public.

Abstract

The current literature shows growing evidence of a link between gender dysphoria (GD) and autism spectrum disorder (ASD). This study reviews the available clinical and empirical data. A systematic search of the literature was conducted using the following databases: PubMed, Web of Science, PsycINFO and Scopus; utilizing different combinations of the following search terms: autism, autism spectrum disorder (ASD), Asperger’s disorder (AD), co-morbidity, gender dysphoria (GD), gender identity disorder (GID), transgenderism and transsexualism. In total, 25 articles and reports were selected and discussed. Information was grouped by found co-occurrence rates, underlying hypotheses and implications for diagnosis and treatment. GD and ASD were found to co-occur frequently – sometimes characterized by atypical presentation of GD, which makes a correct diagnosis and determination of treatment options for GD difficult. Despite these challenges there are several case reports describing gender affirming treatment of co-occurring GD in adolescents and adults with ASD. Various underlying hypotheses for the link between GD and ASD were suggested, but almost all of them lack evidence.”
